MySOS Terms of Service

Allm Co. Ltd (the "Company") provides its "MySOS" service (the "Service") to users in accordance with this "MySOS Terms of Service" (the "Terms").

Article 1 (Application of Terms of Service)

The Terms is applicable to all relationships with the Company with respect to the use of the Service (including use of the Service's app; the same definition shall apply below) You may not use the Service if you do not agree to the Terms.

Article 2 (Definitions)

In the Terms, the following terms shall have the meanings as defined in the items below.

  1. Terms of Service: An agreement based on the Terms under which provision of the Service is received from the Company.
  2. User of the Service: A person who agrees to the Terms and uses the Service.
  3. The service: Means the specialized application software required in order to use the Service with the purpose of registering and managing PHR (Personal Health Record) and medical information of Service Users and their family members.
  4. App use conditions: Conditions for use of the service app separately determined by the Company.
  5. Group code: Means a code given to a citizens' group or fire department group, etc., in a prefecture or municipality with whom the Company has established an agreement in advance. Users of the Service may register with the group code, and once they have entered such group, they can get information only available to the group.
  6. Mynaportal: An online service operated by the government that allows users to check their own information held by government agencies. In addition, users can view and use their own information on the services of private companies through API linkage with services and systems provided by various private companies (the "Mynaportal Linkage").

Article 3 (Formation of agreement on terms of use)

  1. A person who wishes to use this service agrees with the contents of these Terms and Privacy Policy and shall consent by checking the checkbox on the account registration screen on the Service.
  2. The Company shall be able to change this Terms by notifying the user in advance or notifying it. If this agreement is changed and the user continues to use the service, the changed terms and conditions will be applied to that user.

Article 4 (Register an account)

  5. Service Users shall be responsible for the strict control and safekeeping of their e-mail addresses and passwords set by themselves at the time of account registration, etc.
  6. Service Users shall not transfer, succeed, lend, disclose, or divulge their e-mail addresses and passwords to third parties, except with the prior consent of united.
  7. Service Users shall be responsible for damages caused by insufficient management of e-mail addresses or passwords, errors or mishandling in use, unauthorized use by third parties, etc., and united shall not be liable for any such damages except in cases of willful misconduct or gross negligence on the part of united.

Article 5 (Cancel account registration)

In the event that a Service User is found to fall under any of the provisions of Article 8 after account registration, united may, without prior notice, suspend use of the Service, cancel the account, or take any other action deemed appropriate by united against said Service User.

Article 6 (Intellectual Property Rights etc.)

  1. Intellectual property rights, including copyright, and any other rights pertaining to the Service Application related to the Service or provided to the Service User through this Service shall be disclosed to third parties .
  2. License to use this service under this Terms does not permit any assignment or transfer of any rights pertaining to this Service to a User, and the Service User shall not be allowed to transfer the Service beyond the scope set forth in Article 8 We shall not use it.

Article 7 (Prohibited matter)

Article 8 (Contents, etc. of this service)

  6. The Service may include the introduction of products or services of third parties other than the Company, or the provision of services or content.
  7. Responsibility for third party goods or services rests with the third party providing them. Third party goods or services may be subject to the terms and conditions of use and other terms and conditions set forth by the third party providing them.
  8. Article 16 (Non-Guarantee / Disclaimer)

  6. In the event that a Service User suffers damages due to the use or non-use of this Application, the maximum amount of liability for damages assumed by the Company shall be 10,000 yen, except in the case of willful misconduct or gross negligence on the part of the Company.

Article 17 (Exclusion of Antisocial Forces)

  1. The Service User expresses that it does not fall under any of the following items and expresses and warrants that it will not apply in the future as well.

Article 18 (Transfer of rights, etc.)

Article 19 (Competent jurisdiction)

Article 20 (Governing Law)

The establishment, effectiveness, interpretation and performance of the use contract are governed by Japanese law.
